Patient Questionnaire
We are seeking your views on the application from the Llanfyllin Group Practice to close the Llanrhaeadr Medical Centre which is one of three sites they run in the area.
We would like to capture your views before any decision about the future of the branch is taken. If you are completing the questionnaire on behalf of someone else who is unable to do so themselves, then please answer all the questions as they would including the About You details.

Your experience and insights are important. All views received will be analysed and evaluated. A detailed Findings Report with a recommendation will then be produced and considered at a future meeting in public of Powys Teaching Health Board and a decision made.
