The North Powys Wellbeing Programme is on a journey to transform health and wellbeing provision in the north of the county. Part of this work is to ensure that services are suitable for local people, not only for this decade but also for the years to come (although we realise that it's often difficult to predict technological change).

To help us with this work, we'd like to ask you a few questions about your online life - that's if you live in North Powys. By answering our questions, you will give us a better idea of how local people access online services and information and the challenges and barriers they face.

None of the questions are compulsory so please tell us as little or as much as you want to.
