As the needs of our population have changed, more people are living with cancer, mental health, respiratory disease, circulatory disease and musculoskeletal disorders. There is a need to ensure people can ‘Live Well’ through being healthy and active and by accessing early help and support. Living well during adulthood can create huge benefits in older age.
The RPB's Live Well (Disability) Partnership is responsible for taking forward the Regional Partnership Board’s work to strengthen and transform services for people with disabilities in Powys as set out in Powys’ Health and Care Strategy. Here are some of its projects:
Home Support
Home Support is an early help and prevention service for citizens (50+) that enables and provides the support and practical assistance an individual may need in their day-to-day life to live at home with confidence, in good health, independently and safely.
Home support is all about putting an individual in control of their care and support, so they can live at home as independently as possible. To learn more about Home Support, read Early Help and Prevention at Home - Powys County Council
Community Connectors
This project is a key service in delivering the Health and Care Strategy’s Model of Care. It helps clients with care or support needs to achieve ‘what matters’ to them by accessing third sector services and community activities. The service is run by PAVO and is successfully delivering ‘what matters’ to clients in need of support, but also to the health and care professionals who refer clients to the service. Click here to learn more about it.
Return to Home and Progression Model
This new service has successfully enabled a number of individuals who previously had to live out of county due to their support needs, to return home to Powys. These people now live in a local community environment, closer to their families and social networks.
