The Improving the Cancer Journey programme is working to ensure everyone living with cancer in Powys gets the right help and support to achieve what matters most to them.
The aim is to improve the experience and the quality of life for people living with cancer by enabling access to, and coordination of, personalised physical, practical, emotional, spiritual and social support.
In Powys, just under a thousand people are diagnosed with cancer every year. With improvements in cancer care, treatment and drugs, it is now often the case that people are living well, and for longer, with cancer. As a result, it is more important than ever that their health and well-being needs are identified, and met.
We want to make sure that anyone living with, or affected by, cancer has the opportunity to have a supportive conversation, with a Cancer Community Connector. The supportive conversation will identify the things that matter most to the person, at a time that is right for them.
A Macmillan Cancer Support holistic needs assessment will be offered to guide the conversation and create a person-centred care plan. The Cancer Community Connector will offer information and signpost or refer to local support that is best placed to meet the person’s needs.
This support covers a wide variety of topics, such as:
• prescriptions
• health and wellbeing
• practical assistance
• returning to work
• talking about your diagnosis
• counselling
• transport
• money worries
• making a will
• pet care
• local support groups
