Introduction

Key to a sustainable approach in Powys is improving people's chances of living their "best life" at home in their community connected to what matters to them most.

The Challenges ahead - what do we know?

  • The increasing age of the population is driving growing needs for health and care, including in relation to cancer, respiratory & circulatory conditions, frailty, and dementia.
  • More people are living longer with multiple conditions, (two or more chronic health conditions) in a highly rural area, where a third of people live alone. Loneliness can increase the risk of dementia and other conditions. It’s a key reason people seek help.
  • The health, care and third sector workforce is ageing – and there are significant gaps such as home support.
  • Post the COVID pandemic, waiting times for both diagnosis and treatment remain too long. Many people are unable to be discharged from hospital which brings with it the risk of them losing muscle strength and in some cases becoming confused.
  • These complex challenges require both a response to immediate pressures and transformation to ensure sustainability.

See our Better Together Showcards and the feedback posters (right).

The showcards provide information about how we are working together to promote wellbeing and to prevent difficulties escalating to a crisis.

The feedback posters set out the key themes and some quotes from the conversations we had during February and March 2024 when we shared these challenges and tested out our approach with over 170+ people who were representatives from voluntary groups and town and community councils.

The engagement events are part of an ongoing process of dialogue and collaboration between the health board and the council, as well as with other partners and stakeholders. We are committed to listening to and learning from your views, and creating solutions that meet the needs and aspirations of the people of Powys.
